How to Get Started

Fill out the screening form

You found us and now you are ready to work with us!
We first need you to fill out our screening form. This form answers many FAQs and lets us know you have reviewed the main policies before booking your appointment. Additionally, we ensure that you have a concern related to our specialties. If this is unclear, we may reach out with additional questions before sending you a booking link.

Book your initial consultation call

Once you complete the screening form, we will reach out with a link where you can self-schedule your call. Due to limited services and our growing waitlist, we only book initial consultations no more than three weeks in advance. Spots go quickly, the self-schedule link will update whenever appointments become available.
You are required to fill out paperwork prior to the call.

Initial call and scheduling intake

Our quick video introduction is a chance for us to get to know each other. During this call, we make sure that we are a good fit for each other and discuss any concerns. Once we move forward, your therapist will schedule you for a minimum 80-minute intake appointment. You are required to complete the remaining paperwork & questionnaires 48 hours prior to your appointment to avoid cancellation/rescheduling.

Our in-person location is inside the Chamber of Commerce building in the historic Pacific Ave business district of downtown Tacoma. There are two building entrances, one on Pacific Ave and one on Commerce St. Nature-based sessions will be in pre-determined locations around the Pierce County area. Groups and classes take place in King, Pierce, Kitsap, and Thurston counties.

*Relational sessions have multiple intake sessions during a time we call the Intake Period. During this period of time, we are gaining history and understanding of your problems, goals, desires, and developing a plan of action together. Relationship therapy will require 1 initial intake session with all partners, 1 session with each partner individually, and 1 treatment planning session. It is recommended to schedule at least 1.5-2 hours for each of these appointments.

  • Unfortunately, we do not accept insurance, however, we can provide you with a superbill that can be submitted to your insurance company for potential reimbursement. I am considered an Out-of-Network (OON) provider for insurance companies.

    Please check with your insurance for OON options and ask if they reimburse for LMFTAs. You are 100% responsible for working with your insurance if you choose, we do not check for your benefits.

  • There are no sliding scale spots available for individual/relational sessions. There are many sliding scale options available for our groups. Check creativewellbeings.com/events for more information.

  • Cash, debit, and credit are all accepted.

    Payment is due at the time of service unless special arrangements have been made.

  • You must cancel your appointment 48 hours in advance of the scheduled appointment. You will be charged the full amount of the session for missed appointments or failing to cancel 48 hours prior to your appointment. No calls and No shows will be charged the full session fee.

  • Clients are expected to show at the start of the session. If you are more than 15 minutes late, the session will be canceled. This would be considered a no-show and you would be charged a full fee. If you communicate that you are going to be late, the therapist will determine if the session will be conducted. If you cancel two appointments in a row, you may risk losing your spot on our caseload.

  • Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
